You have got reports / letter from the teachers - I presume they comment on the practice papers and other evidence they have and put it into context for the panel?
Say they are longer term; then you would probably expect the school to have noticed this and may be able to comment on the impact of the problem on their school work etc
If it is a short term problem occurring near the test time, the school are more likely to be surprised by the result and produce a report indicating that they were performing at a high level up until the exam.
Though remember - academic evidence is the most important thing

re: the medical issues - much depends on whether they are longer term or more immediate problems:My DC had few medical isssues. Also she had eye problem which is all backed with the optician and GP. Also there is statements form teacher and head teacher. Is there anything else required?
